The Master of Social Work (MSW) will provide comprehensive psychosocial services to patients within our ambulatory care setting. The involves addressing patients' psychosocial needs and enhancing their overall well-being. The social Worker will play a crucial part in facilitating the adjustment of patients and their families to illness, ensuring optimal functioning for both individuals and families. The Social Worker will be expected to excel in the following:
Psychosocial Support: Offer compassionate and evidence-based psychosocial support to children with various diagnoses, including but not limited to medical illness, ADHD, depression, anxiety, and autism spectrum disorders.
Family Assistance: Assist families in navigating the healthcare system effectively. Link them with a range of services tailored to meet their unique needs.
Age-Specific Expertise: Apply age-specific principles of growth and development to your practice.
Collaboration: Collaborate seamlessly with all members of the healthcare team, fostering effective communication and teamwork.
The Master of Social Work (MSW)) will play a vital role in supporting patients and their families by addressing their biopsychosocial needs. Your responsibilities will include conducting assessments, collaborating with the multidisciplinary team, and ensuring effective communication across disciplines.
Responsibilities:
Assessment and Goal Setting:
Conduct thorough assessments of patients and their families' biopsychosocial situations.
Establish clear goals and periodically reassess and modify them as needed.
Crisis Intervention and Support Services:
Provide crisis intervention services for cases involving domestic violence, psychiatric emergencies, child abuse/neglect, family disputes, end-of-life care, bereavement, and Social Determinants of Health.
Assess and help address issues impacting compliance with treatment plans.
Collaboration and Relationship Building:
Work closely with the multidisciplinary team to address patients' mental and behavioral health needs.
Establish and maintain effective working relationships across various disciplines.
Community Resource Assistance:
Assist patients in accessing community resources and support services.
Serve as a liaison between the clinic and external agencies, providing referrals as appropriate.
Resource Awareness:
Stay informed about available community resources.
Provide families with specific information related to these resources.
Case Conferences and Documentation:
Participate in case conferences and team meetings to discuss patient progress and treatment plans.
Maintain accurate and up-to-date documentation in the Electronic Medical Record system, adhering to Medicaid, CMS, and other regulatory requirements.
Additional Responsibilities:
Fulfill any other duties assigned by your supervisor as needed.
Masters degree in Social Work from a program accredited by the Council on Social Work Education (CSWE) and active LCSW licensure required.
