Benefits & Compensation
Salary: $75,000 - $100,000 per year, plus volume-based productivity bonuses. Higher base salary available for candidates with CAC or EMDR certification
Sign-On Bonus: $12,000 paid at time of hire.

Schedule:
5–8-hour days or 4–10-hour days per week with flexibility on preferred schedule.
Role Overview
As a therapist within our primary care team, you’ll deliver integrated behavioral health services that empower patients to make meaningful progress in both their mental and physical health, fostering a holistic approach to well-being and recovery. In this fully integrated, team-based care model, you’ll work alongside primary care providers, nurse practitioners, urologists, surgeons, rheumatologists, pediatric psychologists, and chiropractors, all committed to providing comprehensive, patient care.
Core Responsibilities
Patient Assessment & Treatment:
Create individualized treatment plans focused on incorporating integrative approaches, including mental, physical, and emotional health strategies to support the patient’s overall well-being.
Monitor and document patient progress in the EMR, adjusting treatment as needed.
Collaborative, Team-Based Care:
Coordinate closely with the medical team to identify and provide services to patients who could benefit from behavioral health support.
Establish rapport with referred patients through warm handoffs during PCP visits.
Therapy & Crisis Intervention:
Provide individual, family, and group therapy, along with crisis intervention and short-term behavioral health care.
Educate patients on behavioral health practices and chronic disease management strategies.
Community Connections & Team Integration:
Make referrals to community agencies as needed to support patient well-being.
Participate in clinic meetings and agency staffing to ensure continuity of care and program improvement.
Current Colorado licensure as LPC or LCSW
Ability to relocate to daily commuting distance of Montrose, Colorado
