SUMMARY
Under the general supervision of the Chief Executive Officer, the Chief of Program of All Inclusive Care for the Elderly (PACE) & Senior Services Officer is responsible for the overall development, planning, implementation, and evaluation of the program. Serving as the primary advisor, the Chief of PACE oversees the medical and social services of the program and ensures effective daily operations. This includes managing personnel across administrative, clinical, operational, financial, and programmatic functions within the PACE setting. The Chief of PACE is responsible for all aspects of the PACE Program, including primary care, provider networks, 24-hour care, preventative and therapeutic services, nursing, day health programs, home care, social worker services, transportation, nutrition, and quality assurance.
It is the primary purpose of CHCCC to provide the highest quality of total care possible to the patient population it serves. Such a level of quality depends ultimately on the staff's desire and ability to work together, individually, and as a team. The employee is expected to be professional, punctual, maintain regular attendance, and be cooperative, organized, and enthusiastic at all times.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Additional duties may be assigned with or without prior notice.
Oversees and administrates all facets of the PACE Program, including effective planning and implementation of all program operations and negotiates contracts with outside contractors in accordance with the compliance department.
Develops and manages all external program relations. Represents the PACE Program in the community and on the state and national level and assures positive public relations.
Develops a PACE delivery model specific to the needs of the Central Coast service area.
Secures all necessary PACE funding, including start-up funding, Medicare and Medicaid reimbursement, and overall program fundraising.
Obtains and maintains PACE certification under CMS, as well as any other state or federal laws or rules. Assures compliance with all state and federal regulations and works with appropriate governmental agencies regarding regulatory requirement or changes.
Initiates and implements PACE strategic planning processes and resources both internally and within the local community, including planning for program start up, expansion, and creation of new programs or services.
Oversees and ensures overall PACE program quality and effectiveness, including identifying the need for services and innovative ways to provide care, improving efficiency and effectiveness, and responding to participant and community needs.
Develops and implements written policies and procedures that govern the operation of Central Coast PACE and ensures compliance with PACE provider regulations.
Oversees, directs and supervises PACE key personnel, both salaries and contractual. Reviews and approves the appointment or transfers of all key PACE staff.
Provides leadership in hiring of PACE staff, including determining qualifications, competence, and performance expectations; planning, implementing and monitoring of staffing plans; providing orientation and training and continuing education.
Evaluates performance of employees for compliance with established policies and objectives of the organization and contributions in attaining objectives.
Helps set the stone of the department to ensure morale, teamwork, and that the positive employment culture of the organization is maintained.
Regularly schedules leadership and management, department, and staff 1:1 meeting to provide leadership, development, and a forum for communication to ensure coordination and collaboration in meeting organization and individual goals.
Ensures the financial health of the program, including assisting in preparing an annual operating budget for approval by the Board of Directors, allocating the resources to carry out operational and programmatic activities, reviewing and interpreting monthly financial statements, developing actions to correct areas of concern and developing strategies for further growth and development of Central Coast PACE.
Champions developments, improvements and operation of the Electronic Health Record (EHR) system for the program.
Establishes and maintains an effective system of communication and emphasizes teamwork throughout the PACE program.
Manages the development of the PACE program, including working with other departments and outside agencies.
Managing the process of policy and procedure development and coordination with other departments regarding development of the PACE program.
Provides effective leadership by coordinating and integrating services with other departments as well outside providers and entities.
Responsible for development, implementation, and oversight of contracted provider agreements.
Monitors and controls the budget including processes to monitor operational and capital costs.
Monitors and evaluates the results of programmatic operations against established objectives, quality assurance and service standards and budget/other financial criteria and reports and submits performance plans as indicated to the Chief Executive Officer.
Ensures the program is operating in compliance with all governing regulations and all other applicable regulatory authority and protocols. Ensures the delivery of services and quality metrics comply with state and federal regulations.
Establishes and sustains an advocacy role at the local, state, and national levels for community health centers and the patients they serve.
Demonstrates adherence to and observes all safety policies and procedures, inclusive of infection control rules and regulations.
Demonstrates the knowledge of domestic violence, child, and dependent abuse protocols.
Demonstrates cultural sensitivity and competence.
Maintains and adheres to HIPAA, employee confidentiality, and privileged communications (patient, employee, and corporation).
SUPERVISORY RESPONSIBILITIES
Directly supervises key PACE personnel and may supervise others directly or indirectly. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include interviewing, training employees, planning, assigning, and directing work, appraising performance, rewarding and disciplining employees, addressing complaints and resolving problems.

EDUCATION and/or EXPERIENCE
A Bachelor’s degree in health care administration or related field is required. A Master’s degree in a related field, including, but not limited to, health care administration, business administration, social services, or nursing is preferred.
Four years of work experience in PACE program, long term care, preferably for an at-risk population and in a capitated setting. Proven experience necessary in program development, strategic planning, budgetary oversight, organizational development, resource development, and public relations.
Strong leadership, management and organizational skills, including skills in community collaborations are essential. Strong familiarity with multi-disciplinary programs serving culturally diverse populations is preferred.
LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of an organization. Bilingual in English and another language is preferred, but not required.
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
COMPUTER SKILLS
Experience with word processing, spreadsheets, email, and keyboarding is required. Microsoft Office and Google Suite skills are required. Working knowledge of EHR is preferred.
CERTIFICATES, LICENSES, REGISTRATIONS
Possession of current, valid, unrestricted California Driver's License (Class C) is required. A current CPR (BLS-C) card is required. Must obtain a valid CPR card within the first 30 days of employment if not certified at the time of hire. CHC’s CPR Policy will supersede this job description.
OTHER REQUIREMENTS
Required to pass a criminal history background check and drug screen upon hire. Annual health examination; annual Tuberculosis skin test clearance or chest x-ray; proof of immunity to MMR, Varicella, and Hepatitis B; proof of Tdap vaccine; during current flu season, must provide proof of influenza vaccine or a signed declination form. If declined, a flu mask is mandatory during flu season. Health screening requirements are subject to change based on CDC recommendations and federal, state, and/or local public health ordinances.
