Duke Homecare & Hospice Duke Homecare & Hospice offers hospice, home health and infusion services as well as serves as the home for the Duke Caregiver Support Program. Our Hospice team works closely with a patient's physician to provide comprehensive, individualized care in the comfort their home or at our inpatient hospice facility located in Durham. Our team of nurses, therapists, social workers, counselors, and chaplains help patients and their families manage advanced medical conditions. Duke Home Health provides services in a patient's home that include skilled nursing, physical therapy, occupational therapy, speech therapy, social service, home health aides, and pain and wound management. Our infusion team comes to a patient's home to administer intravenous medications, including antibiotics and chemotherapy, to children and adults. Our nurses, pharmacists, dietitians, and patient services coordinators work under the direction of a patient's doctor to monitor needs and treatment.
Render professional counseling services in a Duke University Health System program to provide individual health programs. Counsel individuals and provide guidance in the execution of program's objective.
Duties and Responsibilities of this Level
Render counseling services to participants in specified programs to include individual and/or group consultation in the execution of the program's function.
Administer, interpret and evaluate the results of any program administered assessment tests.
Counsel individual participants on methods to achieve program prescribed objectives and assist in planning future activities.
Compile and keep records of information concerning services rendered to be used or a service of information for the director of the program.
Organize and conduct recreational activities.
Interview patients and their families for routine admission data; orient patients and families to the facility; provide data input during team discussions and meetings to assist in developing therapeutic plans and objectives.
Interact with patients on a one to one as well as small group basis; assist patients in planning how the treatment plan can be optimally utilized for their benefit.
Provide guidance and instruction to support staff in routine procedures and techniques used to secure confidence and cooperation of patients.

Required Qualifications at this Level
Education: Work requires a Bachelor's degree in Psychology, Sociology, Social Work, Education or a related field.

Knowledge, Skills, and Abilities:

Crisis Prevention Institute (CPI) certification preferred.
Knowledge of procedures and techniques involved in administering routine treatments to patients.
Knowledge of behavioral principles and individual responses to stressors.
Ability to establish and maintain effective working relationships with clients or patients
Ability to work with, express sensitivity and understanding, and secure the cooperation of patients, including maintaining sympathetic attitude towards patients.
Ability to keep calm in stressful situations. Ability to maintain routine records and prepare reports.
Ability to recognize clients or patients in emergency states and seek appropriate assistance.
Ability to maintain confidentiality and other patient rights.
Ability to understand, follow and communicate/relate oral and written instructions and pertinent information accurately.
