This position will work a hybrid schedule which includes a combination of working both onsite on Campus and remote.
DESCRIPTION
For over 30 years, the UCSD Mother Child Adolescent HIV Program has provided comprehensive and integrated HIV care, social services and clinical research to pregnant people, women, children, and youth. A multidisciplinary team of HIV specialists provide medical care and treatment, health education, case management, behavioral health services, and clinical research opportunities.
The Clinical Social Worker I will provide medical case management services to women living with HIV as part of an integrated multidisciplinary team providing outpatient medical and support services. Case management services are delivered with a high level of clinical skill and include comprehensive assessments and care plan development, crisis intervention and safety assessment, and coordination of care. The case manager is expected to provide care that is trauma informed, gender responsive, culturally sensitive, family centered, harm reduction based, evidence informed, LGBTQIA+ affirming, and addresses the mental health and/or substance use needs of our patients.
The case manager should possess a high level of organizational skills, attention to detail, strong written and verbal communication skills, and the ability to work within a multidisciplinary team in a healthcare setting. Other responsibilities may include, but are not limited to, contributing to special projects and quality improvement activities, advocacy, participation in community meetings and committees, networking with referral partners, and co-facilitating groups.
MINIMUM QUALIFICATIONS
A Master's degree from an accredited school of social work; or an equivalent combination of education and experience.
Demonstrated skill in social work assessment and interventions with individuals and families impacted by chronic disease. Strong crisis intervention and counseling skills.
Experience working as part of a interdisciplinary team in a medical or social service setting.
Experience with social work interventions and required documentation in cases of suspected child abuse/neglect, interpersonal violence, elder/dependent adult abuse, substance use and mental health.
Experience integrating theory and practice in the formulation of psychosocial assessment and interdisciplinary treatment plans, the implementation of care plans and the modification of treatment based on outcomes.
Proven experience conceptualizing and writing clear, concise, appropriate psychosocial assessments, care plans, chart notes, and progress reports in social work and medical records.
Experience in thinking creatively and efficiently, to assess, analyze and solve problems.
Ability to deliver care in an ethical, nonjudgmental, culturally sensitive, and non-discriminatory manner.
Demonstrated experience communicating effectively and concisely in English in writing and verbally in formal and informal settings.
PREFERRED QUALIFICATIONS
Knowledge of the physical, emotional, social, financial impact of HIV/AIDS.
Knowledge of electronic medical record; Epic, Appointments Pro scheduling software, AIDS Drug assistance program ADAP eligibility and certification processes.
Fluent (reading, writing, speaking) in Spanish and/or Haitian Creole.

Pay Transparency Act
Annual Full Pay Range: $65,459 - $86,193 (will be prorated if the appointment percentage is less than 100%)
Hourly Equivalent: $31.35 - $41.28
