DESCRIPTION
Under supervision, direction, and guidance of the Registered Nurse, the Rehabilitation Care Partner provides patient-specific care, observation and intervention as directed by unit protocol to high-risk patients. The duties includes and are not limited to assisting with activities of daily feeding, bathing, ambulating, taking vital signs, participating in milieu groups (art and music therapy), and appropriately utilizing restraints on patients. Ensures patient care guidelines are followed in order to reduce and prevent self harm. Performs all other related support functions as required.
MINIMUM QUALIFICATIONS
BLS/CPR certification at time of hire with commitment to get BART within six (6) months of hire date.
A minimum of three (3) years of directly related experience.
A minimum of one (1) year of experience working with vulnerable and behavioral high risk populations, specifically trauma, brain injured, incarcerated, homeless, and/or substance dependent patients.
Commitment to complete 16-hour UCSD CPI/behavioral management/de-escalation training within six (6) months of hire.
Completion of 4-hour Trauma Onboarding course upon hire.
Completion of O-Log training or similar TBI cognitive tracking scale upon hire.
Completion of 8-hour training with PT/OT on safe mobility for traumatically injured patients upon hire.
Must be able to communicate in English and possess demonstrated positive written skills.
PREFERRED QUALIFICATIONS
CNA Certification.
Prior experience as a CNA, Psych Tech, Nursing Student, ED Tech, or Trauma Tech.
Epic, UCSD Paging & Scheduling Systems.
Bilingual: Spanish/English.
Experience in care of the acutely brain injured patient and/or the rehabilitation of brain injured patients.
Experience in the care of traumatically injured patients with psychiatric illness, primary brain injury, and/or substance abuse comorbidities.
Experience working with vulnerable populations.
CPI training and/or de-escalation experience.
SPECIAL CONDITIONS
Employment is subject to a criminal background check and pre-employment physical.
Flexible to work variable hours including overtime, nights, weekends and holidays.
Initial and annual training requirements for the Rehabilitation Care Partner are subject to modification by the 5 West unit.
New hires who fail to complete initial training requirements will be released from employment.
Pay Transparency Act Full Salary Range: $51,532 - $64,039 Annually
