The Licensed Professional Mental Health Counselor, Coordinator coordinates Suicide Prevention efforts in the Sheridan VA Health Care System, is a SME on suicide prevention, functions as part of the Multidisciplinary Treatment Team comprised of professional and Para-professional Mental Health Staff and is involved in case assessment and treatment and discharge planning. The incumbent offers Psychosocial Assessment and Treatment to eligible veterans and their families and/or significant others. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Hold a master's or doctoral degree in: Clinical Mental Health Counseling; Clinical Rehabilitation Counseling; Clinical Mental Health Counseling and Clinical Rehabilitation Counseling; or a related field, from a program accredited by the Council on Accreditation of Counseling and Related Educational Programs (CACREP). Examples of related mental health counseling fields include, but are not limited to Addiction Counseling; Community Counseling; Gerontology Counseling; Marital, Couple, and Family Counseling. CACREP defines the date when graduates are considered to have graduated from a CACREP accredited program. Additional information may be obtained from http://www.cacrep.org/directory/ Licensure: Persons hired or reassigned to LPMHC positions in the GS-0101 series in VHA must hold a full, current, and unrestricted license to independently practice as a Licensed Professional Mental Health Counselor, which includes diagnosis and treatment. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Licensed Professional Mental Health Counselor (Program Coordinator), GS-12 Experience, Education, and Licensure: In addition to the basic requirements, candidates must have at least one year of progressively complex LPMHC experience equivalent to the GS-11 grade level. Senior LPMHCs must have five years of post-licensed experience that demonstrates possession of advanced practice skills and judgment, demonstrating progressive professional competency and expertise and be licensed to provide clinical supervision to trainees or unlicensed LPMHCs. In addition to meeting the KSAs for the GS-11 grade level, the candidate must demonstrate all of the KSAs below: 1. In depth knowledge of the program coordinated, and demonstrated knowledge and ability to write policies procedures and or practice guidelines for the program. 2. Ability to make key decisions in the hiring and identification of top candidates. 3. Ability to provide leadership in facilitating the process of coordinating program consults, assessments, admissions, care assignments and discharges. 4. Ability to provide subject matter consultation to other colleagues and trainees within the field of counseling working on an interdisciplinary team. LICENSED PROFESSIONAL MENTAL HEALTH COUNSELOR QUALIFICATION STANDARD GS-0101, Veterans Health Administration The full performance level of this vacancy is 11.The actual grade at which an applicant may be selected for this vacancy is range GS-12. Physical Requirements: This position provides direct services to patients and their families through casework. Incumbents must be physically and mentally able to efficiently perform the essential functions of the position without hazard to themselves or others. Normal work of the position requires visual acuity (with the ability to read typewritten characters without strain, corrective lenses permitted), keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially moderate levels of lifting and carrying (15-44 pounds). The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. ["- Maintains a list of patients determined to be currently high-risk for suicide and monitor their care to assure that needed services are being received by telephone contact.\n- As needed or appropriate, consults/collaborates with providers of high-risk patients for purposes of education and facilitating best practice. May provide direct patient care for same purpose.\n- Provides training to clinical and non-clinical staff in the Medical Center, Community Based Outreach Clinics\n(CBOC's) and community about risk factors, warning signs, treatment options and resources.\n- Promotes the use of evidence-based interventions and strategies for suicide prevention in the Medical Center.\n- Chairs the RCA (Root Cause Analysis) Suicide Team by performing individual case reviews, assisting with preparation of quarterly aggregate summaries and facilitating recommended system improvements.\n- Responds to consults from the Veteran Crisis and Military Hotline as soon as possible and contacts veterans to engage them in treatment and reports back to the Hotline Staff about the results of interventions.\n- Facilitates assessment of Suicide in the facility and Suicide Risk Assessments.\n- Facilitates implementation of reporting high risk patients through use of the Suicide Behavior/Overdose Report.\n- Facilitates the implementation of Category 1 Flags per Central office Directives.\n- Implements Guide Training throughout the Sheridan VA Healthcare System and in the community and sends evaluations to the Center of Excellence.\n- Act as a liaison to community organizations and veterans organizations.\n- Provides Tracking Reports to the Center of Excellence for data compilation once a month.\n- Assuring that all veterans on the facility tracking list are being followed closely by the appropriate provider on a regular basis as determined by the coordinator and the provider.\n- Tracking any no-show appointments by these patients and following up with the patient as soon as possible to reschedule the appointment.\n- Maintaining awareness of suicide treatment options and making recommendations to the patient's provider when new options become available.\n- Obtaining any relevant HIPAA clearances to allow transmittal of facility data on suicide attempts and individuals at risk to the VISN 19 MIRECC for trending and quality assurance purposes; and transmitting the data following cyber security requirements as applicable for this sensitive data.\n- Assuring that all of these patients have a 24-hour resource number to call and ways to get assistance if an immediate crisis should occur.\n- Provides suicide prevention training to Veterans, staff and community members.\n- Compiles data, completes required Local, VISN and National reports, develops plans to facilitate meeting required performance measures (SAIL, Director's Performance etc.).\n- Works across service-lines to facilitate compliance with VHA directives and initiatives related to Suicide Prevention.
