The primary location is remote with service delivery being in National City at the CARE Center, though recruitment may occur around the South East San Diego area.
The hired candidate will work around 20 hours per week.

The new Herbert Wertheim School of Public Health and Human Longevity Science is dedicated to health and social equity and has over 80 primary salaried faculty, 25 secondary faculty, 200+ personnel, 200 postgraduate students, and 550 undergraduate majors. The School includes activities in the Public Health disciplines of Behavioral Medicine, Biostatistics & Bioinformatics, Epidemiology, Global Health, Health Policy, Health Equity, Public Mental Health, and Environmental and Climate Health. The School administers two PhD programs (Public Health and Biostatistics), a Master of Public Health professional degree program, an MS in Biostatistics, the only Health Sciences undergraduate program (BSPH), an MAS in Leadership of Healthcare Organizations run in conjunction with UCSD Extension, and a Preventive Medicine residency program.
The School has strong collaborative research programs facilitated by Research Centers of Excellence focused on Wireless and Population Health Systems, Health Promotion and Equity, Women's Health, and Tobacco Control, and has extensive collaborations with the School of Medicine, Skaggs School of Pharmacy, Jacobs School of Engineering, the Scripps Institution of Oceanography, Moores Cancer Center, the Altman Clinical and Translational Research Institute, and with other partners across the UC San Diego campus. Partnerships with the community and health agencies are fundamental to the School. The School houses five recharge cores providing services to UCSD and external customers, and the School's faculty and academics hold ~$125M total awarded grants and contracts.
The UCSD RELINK Program provides service navigation support to adults ages 18+ who have ever been incarcerated in California state prisons and reside in the San Diego County region. Service Navigators meet once-weekly with the client to support and facilitate engagement with healthcare and social services tailored to the needs of the client. The RELINK Program Service Navigation approach is strengths-based and collaborative and uses motivational interviewing techniques. The program also seeks to improve access to and use of health and social services among clients and to assist them in meeting their basic needs.
Under general supervision, the incumbent will independently work as a Reentry Community Service Navigator. The geographic focus of this position is the Southeast and Southern San Diego region.
The incumbent is responsible for the following duties: Assist with the recruitment of clients via presentations at community partner organizations.Conduct phone and text outreach to potential clients in order to engage them in the program.Perform orientation appointments with all new clients (this includes an informed consent process, administration of a needs assessment survey, and a collaborative discussion with the client to identify clients' strengths and needs).
Guide and support each client to identify, access, and utilize needed healthcare and other supportive services. Provide client advocacy, make appropriate referral in cases of complex problems, and brokerage to obtain housing, financial aid, vocational resources, and other health and social services. Provide mentorship and support in soft-skill development. Document client appointments, referrals made, successful use of referrals, content of sessions, goals and successes after each session. Maintain a supportive and stable relationship with the client for a period of at least 3-months. Meet regularly with program administrators and provide feedback about the content of the program for future program improvement. Serve as a liaison between the UCSD RELINK Program to other programs in the community Special duties or assignments may be requested.
Four (4) years of related experience, education/training, OR a Associate's degree in related area plus two (2) years of related experience. Related experience includes serving formerly incarcerated persons or justice involved adults.
Experience/Interest/Dedication to working with formerly incarcerated adults.
