The PCMHI Senior Social Worker provides brief assessment and intervention support to the Patient Alignment Care Team (PACT), with an emphasis on integration of mental health services within primary care. Incumbent incorporates complex multiple causation in differential diagnosis and treatment of Veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Incumbent employs a high level of skill and expertise to establish and maintain effective therapeutic relationships with Veterans and/or their families. Incumbent independently works with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems. Provide a full range of assessment and evaluation services appropriate to the primary care setting, especially in the areas of emotional and behavioral problems Provide a range of advanced interventions consistent with training and expertise, with special emphasis on brief and evidenced-based interventions found to be effective in primary care settings; The incumbent will model the national PCMHI competencies at the advanced level of practice and provides consultation to less experienced social workers. Function as a subject matter expert. The senior social worker will perform at least one or more of the following; process improvement tasks, supervision of social workers for advanced practice licensure or the graduate trainee program, act as a mentor for less experienced social workers, participate or chair committees or groups that impact social work practice and/or lead rollout of evidenced based practices. Actively participates in program planning and development, advising the PCMHI Program Manager of shifting trends with caseloads and identifying the need for and making recommendations on program procedures or policy changes based upon work experience. Responsible for improving professional skills, participating in planning and executing specific staff development projects and attending seminars and institutes, and participating in training programs of other disciplines as requested. Expected to act as surrogate to the Social Work Supervisor, for CPRS purposes, cosigning notes/consults/orders and providing clinical guidance when appropriate or necessary. Verifies the veteran's status, eligibility for VA medical care, and eligibility of program participation. Coordinates care including serving as an advocate on behalf of veterans to ensure comprehensive service delivery and linking and referring veterans to the VA Medical Center Services, VA regional Offices and/or community agencies.
