JOB SUMMARY: Facilitates scheduling of outpatient specialty appointments for patients who are unfunded and out of network. Interfaces with internal customers to determine whether patient will continue specialty follow up care at Vanderbilt or will be connected to a community resource. Demonstrates a strong internal and external customer focus. Work under the direction of the Network Navigators.
Assists team in identifying appropriate specialty follow up appointments necessary to patients' safe discharge.
Adheres to standard workflow that identifies process and decision points. And assists in identifying service gaps and/or system issues that interfere with timely/appropriate discharge. Demonstrates critical thinking skills as regards the process flow associated with scheduling follow up appointments.
Demonstrates ability to collaborate with physicians, Network Navigators, Financial Counselors and other team members as well as community providers.Identifies, maintains, and updates information on community agencies and resources necessary for patients' continuing care.Contributes to the organization by actively participating in process improvement initiatives and shared decision making.Demonstrates ability to document appropriate information as guided by standard workflow.

Able to train and educate by setting the example and giving technical instruction. Demonstrates acute knowledge of industry trends and emerging technologies. Attends industry seminars and forums on a regular basis.
FAMILY ADVOCACY (INTERMEDIATE): - Demonstrates mastery of family advocacy in practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering results without requiring support and instruction from others. Able to train and educate by setting the example, giving instruction, providing leadership, and generally raising the level of performance of others while on the job.
