As a clinical employee that serves the needs of young, middle and older adults in Behavioral Health Services, the position of a care coordinator is responsible for providing psychotherapy and care management services. The position requires the ability to promote the coordination, collaboration and continuity of care from admission through discharge and post-discharge. The position necessitates leadership skills with the ability to direct and evaluate all aspects of patient care, and to assure that care is delivered a timely, effective and efficient manner. The position interacts with payors to determine the appropriate length of stay, care interventions and discharge plan, utilizing the resources available within the patient’s care network. The position requires an ongoing knowledge of managed care and how best to deal with payors. Excellent organizational skills, the ability to consistently multitask and communicating well with others are critical aspects of this position. The ability to interact, educate and develop a therapeutic alliance with families of patients is essential.
Education or Formal Training 1. 2-3 years of psychiatric patient care coordination experience strongly preferred. 2. Master’s Degree in Social Work, Family Therapy, Counseling, Psychology or Psychiatric Nursing or related field is required for non-licensed Therapists.
Special Qualifications (licensure, registration, etc.) 1. Must be licensed or eligible for licensure. 2. CAC II or III; experience in a Chemical Dependency Treatment setting, preferred. 3. Colorado LPC or LCSW Licensure required, LCSW preferred, for Licensed Care Coordinator position. 4. Colorado Registered Psychotherapist, LPC Candidate or Licensed Addictions Counselor required within 3 months of hire for non-licensed Care Coordinators.
Knowledge, Skill and Ability 1. Knowledge of family systems theory and its application in the short-term setting. 2. Ability to assess and evaluate a wide range of psychosocial impairments within a patient population 18 through the continuum of life. 3. Knowledge of current state and federal laws relating to mental health (27-10, JCAHO and ADAD). 4. Knowledge of Standards and Guidelines specific to BHS. 5. Knowledge of Medicaid/Medicare regulations. 6. knowledge of and ability to implement the PASSR process. 7. Knowledge of reimbursement systems and network systems used in the process of Utilization Review.
Experience 1. 2-3 years’ experience in a psychiatric setting inpatient and/or outpatient. 2. Recovery experience preferred.
WORKING ENVIRONMENT/PHYSICAL ACTIVITIES
1. Functional physical demands include manual dexterity and fine motor skills. The following senses will be needed for essential duties of the job: speech, vision, hearing, smell & touch. 2. Accomplish tasks while working under pressure of consistent deadlines. 3. Work in artificial light and sits for substantial periods of time. 4. Ability to accomplish tasks while being consistent with ENCORE behavioral standards. 5. Ability to work in high-stress noisy environment.
1. Ability to maintain internal contacts with physicians and employees, ability to act as resource and motivate these individuals. 2. Interdepartmental relationships throughout the hospital. 3. Ability to communicate effectively with public and self-help organizations regarding available resources at Boulder Community Hospital and the community at large. 4. Ability to coordinate services with medical staff, treatment programs and third-party payors. 5. Professional relationship with patients and families. 6. Ability to communicate effectively with community resources related to disposition planning.
