Rutgers, The State University of New Jersey is seeking a Part Time Habilitation Counselor for the Outpatient – Specialized Case Management and Outreach Services (SCMOS) Program at Rutgers University Behavioral Health Care.
The purpose of the Habilitation Counselor position is to provide community based outreach targeted case management services that include assessment, treatment planning, linkage and monitoring.
Among the key duties of this position are the following:
Provides assessment, treatment planning, monitoring and linkage services to a caseload of individuals served.
Provides targeted case management services to assist individuals to gain access to needed medical, social, education, mental health, addiction, co-occurring and other services.
Collaborates effectively with other departments, staff, referral sources, other service providers and social support networks to provide services and rehabilitative care for consumers and families.
Assists individuals in effectively managing crisis/dangerous situations and is able to formulate a crisis/safety plan, as measured by supervisory observation.
Provides services that are Person Centered that assure that the person's expressed needs and strengths drives the service plan and service delivery.
Minimum Education and Experience:
Bachelor's Degree in the mental health field or other appropriate human services related field plus one (1) year of related experience.
Required Knowledge, Skills, and Abilities:
Possess own vehicle.
Experience with community based service provision to the population.
On-call responsibilities as assigned.
