Rutgers, The State University of New Jersey is seeking a Clinical Care Coordinator for the Department of Cancer Health Equity at Rutgers Cancer Institute of New Jersey.
The primary purpose of this position is to provide direct, high-quality tobacco cessation counseling and care to patients of the Rutgers Cancer Institute of New Jersey and affiliated RWJBarnabas Health System Cancer Center sites with the goal of increasing the volume of high-risk oncology patients receiving tobacco treatment services. Clinical services and patient care provided by this position include clinical assessment, individual and group treatment, and telephone/telehealth consultation for patients seeking treatment for tobacco dependence, development of the clinical plan for tobacco dependent patients to improve integrative care and reduce/eliminate the tobacco burden and provide evidence-based tobacco treatment services, and education to patients and their families who currently use or have a history of tobacco use. This position also collaborates with interdisciplinary team members for program referrals, care improvement, and to support the integration of evidence-based practices based upon the US Public Health Services Clinical Practice Guidelines and other scientific evidence and clinical guidelines across multiple affiliated cancer center sites in New Jersey. Additionally, this position coordinates tobacco treatment efforts between the Rutgers Cancer Institute of New Jersey and Cancer Centers within the RWJBarnabas Health System to improve delivery, consistency, and effectiveness of evidence-based tobacco treatment services, referral of patients to related cancer screening and prevention programs.
Among the key duties of this position are the following:
Provides educational services and programs for patients, families and staff within the framework of the department.
Responds to inpatient and outpatient referrals for tobacco treatment and effectively collaborating as clinically indicated with other staff members, referral sources, community resources, service providers, case management organizations, families and support systems.
Provides direct cessation services for tobacco dependence in inpatient, outpatient, group, in both in-person and telehealth settings based on sound clinical judgment and established clinical practice guidelines with empathy and concern.
Collaborates with other team members to improve the delivery of care.
Assisting in development, implementation, and ongoing evaluation of standardized tobacco treatment dependence programs.
Minimum Education and Experience:
Minimum of Bachelor's Degree in Nursing (BSN) or a related health care field.
A minimum of two years of full-time experience in oncology or experience in a hospital-based practice.
Equivalent education, experience and/or training may be substituted for the degree requirements.
Required Knowledge, Skills, and Abilities:
Effective team leadership and interpersonal skills.
Computer literacy, typing, working at multiple workstations in a shared environment, ability to speak clearly, write proficiently.
Computer literate with proficiency and working knowledge of database and reporting tools such as Microsoft Word, Excel, Access and PowerPoint.
Detail-oriented and have excellent writing and editing skills.
Outstanding interpersonal skills, and the ability to function in a team environment.
Preferred 2 years' experience in tobacco dependence treatment.
