1. Provides clinical care management for individual students requiring assistance, support, and follow up contact. Monitors internal and external wait times to ensure timely access to services. 2. Interacts with contacts such as patients, patients' families, clinicians, medical staff, and administrators in regard to treatment of individual patients and implementation of clinical programs. 3. Serves as the primary referral coordinator between embedded mental health counselors, and other clinical and academic departments, and MHC. Assists in reassignment and functions as student liaison to ensure a good fit. 4. Assesses clinical needs of students and assists with the development of appropriate disposition plans, in collaboration with MHC and MHC clinicians. Maintains patients' charts and reports. 5. May provide brief clinical assessment, evaluation and interventions while students are in process of connecting with providers. 6. Coordinates external referrals for students from YH MHC to Magellan, in collaboration with senior clinical staff. 7. Develops and maintains a database and systems to track referrals. Identify students requiring assistance, assess student feedback, and report utilization metrics. 8. Develops and maintains database of local, national, and international mental health resources. 9. Authorizes referrals and communicates with Member Services, Claims, and Billing to ensure coverage of appropriate services. 10. Functions as the primary contact for students and clinicians in regards to all external services. May coordinate with external mental health facilities and/or providers to find and facilitate placement or services for students requiring care outside of the Yale. 11. Establishes positive relationships with patients referred to external services. Supports students through the referral process and ensures timely and appropriate scheduling with community providers. system. 12. Establishes and maintains strong working relationships with department leadership, clinicians, students, and university stakeholders. 13. Adheres to the expectations of strict privacy and confidentiality of health information. 14. Provides educational and consultation services to undergraduate residential colleges, graduate and professional schools. Develops materials for educational programs and seminars for professional staff. 15. May perform other duties as assigned.
