Make a Difference in the WakeMed Community as a Supervisor, Case Management – Community Case Management
As a Supervisor of Patient Case Management, you will assist with managing the day-to-day operations of the Patient Case Management Department. You will work with Patient Case Management leadership and the Patient Case Management Physician Advisor to carry out department goals. You will serve as a clinical resource to staff of the Patient Case Management department, physicians and nursing leadership. You will also collaborate with physicians, nursing, members of the interdisciplinary team, Patient Financial Services, Patient Access and other departments to ensure services are being provided in a timely/quality manner.

Bachelor’s Degree in Nursing or Master’s Degree in Social Work Required
Master’s Degree in Nursing Preferred
3 Years of Clinical – Case Management or Clinical – Discharge Planning Required
And 1 Year Supervisory Preferred
Registered Nurse or Licensed Clinical Social Worker or Licensed Professional Counselor or Licensed Clinical Addictions Specialist or Certified Case Manager Required
