Provides triage, assessment, determination of appropriateness for admission or referral, treatment, education, utilization, provision of resource information and disposition planning to patients and families, in particular those patients with mental health, psychiatric and substance abuse diagnoses.
**This position is for a "mobile assessor" for a co-responder with local law enforcement**
Work Schedule: Full Time Days
Develops individualized treatment plans, provides care/case management and directs appropriate disposition. Supports all activities with appropriate documentation.
Provides group and individual counseling services using accepted psychotherapeutic models and interventions, including integrative healthcare options. Assesses, collaborates and coordinates patient's care and discharge planning needs and readiness for discharge with colleagues in Care Management and Medical Social Work and other members of the care team.
Coordinates psychosocial care as needed with third parties and community agencies. Initiates psychiatric holds and Emergency Commitments as appropriate. Responds to referrals, incidents, or problems, provides coverage as assigned, assists staff with cases as needed.
May verify benefits and supports financial counseling. Orients patients and their families to the requirements, policies, and goals of the proposed treatment program. May provide clinical supervision for LCSW, LCSW candidates, LSW; as well as MSW, MFT, PC, and other students and support staff in relevant disciplines as assigned.
Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.
Minimum Required Education: Master's degree in Social Work.
Required Licensure/Certification: State licensure as a Licensed Clinical Social Worker (LCSW). Minimum Experience: None Required.
The pay range for this position is: $27.61 - $41.41 / hour
