JOB SUMMARY/ESSENTIAL JOB FUNCTIONS: Under the direction of the Program Administrator, this position exists to support and assist caregivers in determining specific needs and obtaining needed supports with projects resources through the network and /or other providers. This position provides hands on contact with individuals and families through home and office visits.

DUTIES AND RESPONSIBILITIES:

1. Identifies participants eligibility through intakes done daily by phone, walk-ins, and -referral from other agencies.
2. Conducts home visits to assess family environment-include monthly and /or quarterly contacts.
3. Develops an individualized support plan - a case plan developed with the for the family to receive appropriate services.
4. Determines specific needs with families.
5. Re-assesses families for eligibility after completion of a year of services through the relative caregivers' program.
6. Provides monthly or quarterly face to face meetings or phone contact to program participants.
7. Maintains accurate case notes on clients from initial contact up to and ending with case closure; submits accurate monthly reports denoting the number of client contacts, cases opened and closed, and an overview of services provided.
8. Collaborates with outreach agencies to secure additional resources to gain greater family stability.
9. Attends Individualized Education Program (IEP) workshops pertaining to family preservation and job training.
10. Maintains necessary documentation and reports data and demographics of services/families served by the program.
11. Performs other related duties as assigned.
