UW Neighborhood Clinics (UWNC) is a network of community-based clinics throughout the Puget Sound region. The clinics offer a wide spectrum of primary care services, as well as secondary care, from pediatrics to geriatrics. Urgent care services are offered seven days a week at select locations. The clinics are consistently recognized for high quality standards by the Accreditation Association of Ambulatory Health Care and the National Committee for Quality Assurance Patient-Centered Medical Home Program.

UWNC - Northgate Clinic has an outstanding opportunity for a full-time (100%, 40 hours per week), day shift, SOCIAL WORKER. As a member of a health care team, the Social Worker will provide behavioral health services (BHIP) as well as generalist social work services to primary care patients focusing on providing direct patient care, research, and teaching.

RESPONSIBILITIES

Provide behavioral health clinical services to patients receiving care. Social work interventions include psychosocial assessment, crisis intervention and other clinical social work services

Provide evidence-based, mental health counseling in accordance with the BHIP model.

Screen and assess patients for common mental and substance abuse disorders.

Provide patient education about common mental and substance abuse disorders and available treatment options.

Support and closely coordinate mental health care with the patient’s primary care provider and UWNC consulting psychiatrists.

Provide information and referral, advocacy, and other generalist social work services.

Performs related duties as assigned/required. Ability to function in a fast-paced, intensive, and time pressured work environment.

REQUIREMENTS

A Master of Social Work Degree from a program accredited by the Council on Social Work Education
AND

One year of full-time social work experience in a health care setting or equivalent. Experience can include a practicum placement as part of a social work training program and/or employment experience.

LEGAL REQUIREMENTS
Within 90 days of hire must hold one of the following credentials issued by the state of Washington:

Agency Affiliated Counselor, or

Licensed Social Work Associate -Advanced, or

Licensed Social Work Associate - Independent Clinical, or

Licensed Advance Social Worker, or

Licensed Independent Clinical Social Worker

DESIRED

Three years of full-time professional social work experience in a health care setting or equivalent

Eligible to be a social work practicum instructor for the University of Washington School of Social Work.

Licensed Independent Clinical Social Worker (LICSW) at time of hire.
