Senior Habilitation Counselor

Position Summary:

Rutgers, The State University of New Jersey is seeking a motivated Senior Habilitation Counselor for the Outpatient Treatment – Certified Community Behavioral Health Clinic (CCBHC) Case Management at Rutgers, University Behavioral Health Care (UBHC).

The purpose of the Senior Habilitation Counselor position is to provide a lead role for the CCBHC Case Management staff and to provide on-site and community based case management and supported employment services to individuals receiving treatment services through UBHC's Outpatient Certified Community Behavioral Health Clinic (CCBHC) services.

Among the key duties of this position are the following:

• Takes the lead with identifying Quality Improvement (QI) opportunities and leads unit QI activities.
• Supports individuals to gain access to needed medical, social, education, mental health, addiction, co-occurring and other services.
• Provides services that are person centered that assure that the person's expressed needs and strengths drives the service plan and service delivery.
• Collaborates effectively with members of the department and with external departments, staff, referral sources, other service providers and social support networks to provide services and rehabilitative care for consumers and families.

Minimum Education and Experience:

• Bachelor's Degree in the mental health field or other appropriate human services related field plus two (2) years of relevant mental health experience.
• Equivalent education, experience and/or training may be substituted for the degree requirement.

Preferred Qualifications:

• Previous community-based outreach experience.
• Bilingual skills (English/Spanish).

Physical Demands and Work Environment:

• The employee can expect to lift up to 30 pounds.

Special Conditions:

• May work at least one (1) evening per week and one (1) weekend day per week.
• Must possess own vehicle or alternate means of transportation to outreach individuals served and attend internal and external meetings.
