This position is funded by Inpatient Pharmacy, but will service an Outpatient setting
Under general supervision, the Staff Pharmacist II is responsible for provision and documentation of medications and medication-related services to patients, according to standards established by the Department of Pharmacy. This position will provide medication therapy management services for patients in the Ambulatory Care clinics to assist existing Ambulatory Care Pharmacists for vacation, leave, and other coverages. Most notable clinics likely to require coverage may include but are not limited to Cardiothoracic and Abdominal Transplant, Hepatitis C, Inflammatory Disease, and Population Health virtual MTM.
The pharmacist will also be responsible for provision and documentation of medications and medication-related services to patients according to the standards established by the Department of Pharmacy, as applicable. The pharmacist may serve as preceptor to the pharmacy students and residents as assigned. May also participate in Medical Center and Departmental committees and promote institution-wide medication use policy. Other activities may be assigned at the discretion of managers and supervisors.
Depending on experience of the selected candidate, this position will be hired at the Stf Pharm1 or Stf Pharm2. Less than four (4) years experience qualifies as Stf Pharm1; Four (4+) years of experience qualifies as Stf Pharm2.
Doctorate of Pharmacy Degree or equivalent clinical experience
Registered Pharmacist - issued by the state of California (RPH)
Incumbents can meet the minimum experience qualifications for this position in 1 of 3 ways: 1) Possesses a PGY 2 or 2) Possesses a PGY 1 plus one (1) year of pharmacist experience, or 3) Possesses four (4) years of pharmacist experience.
Pharmacy residency or equivalent clinical experience.
Must be able to work various hours and locations based on business needs.
