Will join a team of infectious disease physicians and pharmacists. Team oversees antimicrobial use throughout UC San Diego Health inpatient facilities and assists in outpatient settings including infusion center. The antimicrobial stewardship team provides oversight of the restricted antimicrobial formulary which requires high cost, high toxicity, broad-spectrum, and on-shortage antimicrobials to be used with stewardship or Infectious Diseases approval.
We oversee the antimicrobial formulary, deciding what drugs will be regularly purchased and stocked at UCSDH. Position collaborates on various practice guidelines involving antimicrobials including sepsis guidelines, surgical antibiotic prophylaxis guidelines, pneumonia guidelines, and C diff treatment guidelines, etc. Collaborates with Electronic Medical Record (EMR) medication process team to optimize ordering for antimicrobials. Participates in collaborations for multidisciplinary projects and in quality improvement projects at UCSD and UC Health.
Clinical ID Consultation: Pharmacist will provide patient specific consultation to pharmacists and providers as requested. Teaching: Pharmacist provides clinical training to pharmacy students, and residents (PGY1 & PGY2). The Department conducts a PGY2 Infectious Disease Pharmacy Residency program.
Doctor of Pharmacy Degree (Pharm D) or equivalent training.
Post graduate year 1 [PGY1] in Pharmacy Practice; or equivalent experience.
Post graduate year 2 [PGY2] in area of specialty practice; or equivalent experience in specialty area.
Registered Pharmacist - issued by the state of California (RPH).
Board Certification in Pharmacotherapy with added qualifications in Infectious Disease and/or Board Certification in Pharmacy Infectious Disease Practice.
A minimum seven (7+) or more years of relevant experience or two (2) years of Residency training, plus 1yr Clinical ID practice.
Demonstrated experience and proven expertise of pharmaceutical theory in a specialty theory.
Knowledge of developments in research and design in area of specialization including new or reformulated products being developed by medical and pharmaceutical researchers.
Completion of PGY2 Pharmacy Residency in Infectious Disease.
Clinical training experience of pharmacy students, and residents.
Previous experience providing pharmaceutical care in the inpatient and outpatient settings.
Experience with Epic EHR functionality.
Must be able to work various hours and locations based on business needs.
