PGY-2 Ambulatory Care Residency

Lipscomb University College of Pharmacy had developed a unique PGY2 Ambulatory Care Residency position collaborating with many different healthcare providers and organizations. The resident will have the opportunity to practice in a HRSA funded clinic for underserved patients and a private practice physician group. The resident will also have the opportunity to have electives in academia and spend a portion of the their time in an private endocrinology practice. The Clinic at Mercury Courts is an interprofessional collaboration between Urban Housing Solutions, Vanderbilt University School of Nursing (VUSN), and Lipscomb University College of Pharmacy and Health Sciences. VUSN applied for and received a 1.5 million dollar federal grant through the Health Resources and Services Administration (HRSA) Division of Nursing to begin September 2012. The nature of the interprofessional collaborative practice involves a physician, family nurse practitioner, pharmacist, and professional counselor. The pharmacist role in this unique practice setting involves formulary management, medication counseling, chronic disease management, health promotion and preventive activities for patients, collaboration with Interprofessional Collaborative Practice (ICPC) team, and clinical preceptor for students from six different health professional programs that rotate through the clinic on a routine basis. The clinic itself serves disadvantaged and vulnerable populations of Nashville. Maury Regional Medical Group, an affiliate of Maury Regional Medical Center is a group of 100 multi-specialty providers. The group specialties include internal medicine, family practice, neurology, gastroenterology, endocrinology, women's health, pulmonary disease and cardiology. Under a collaborative practice agreement the pharmacist has the opportunity to manage several chronic disease states including diabetes, hypertension, hyperlipidemia and heart failure. The pharmacist is an integral parts of a multidisciplinary team approach within the medical home model. The pharmacist has a role of growing clinical pharmacy services and improving patient outcomes across a diverse spectrum of patients. The resident will have an opportunity to learn management skills, establish revenue streams for clinical services, and impact population health. New to the program this year are opportunities in transitions of care as well as telehealth. Through collaboration with LUCOP, the resident will complete a teaching certificate program in which they will deliver didactic lectures and facilitate active learning sessions, supervise pharmacy students including being primary preceptor for a minimum of one month. At the practice site they will present patients, lead journal club presentations, participate in committees and practice meetings. Additionally, the resident will complete a residency research project with the opportunity to present their research at a local, regional, and/or national meeting.
