1. Provides direct assessment and treatment to individuals, groups, and families with mental health and substance use conditions or behavioral problems.2. Completes psychosocial assessments, mental status examinations and diagnostic assessments, and translates this information into multidisciplinary treatment plans.3. Provides individual and group evidenced-based interventions for clients and families.4. Anticipates problems and crises and intervenes to de-escalate and manage situations.5. Evaluates progress and presents findings and recommendations for changes in interventions at treatment team meetings.6. Coordinates and provides clinical case management services, including referrals to appropriate levels of care.7. Provides crisis intervention, clinical triage, and referrals to other services as necessary.8. Monitors discharge readiness of clients and compiles discharge related assessments and summaries.9. Collaborates with families, medical and clinical staff, and other health and social service providers in delivering services to clients.10. Utilizes electronic medical record system for entry of requisite assessment, incident reports, and session documentation.11. Performs other duties as needed.
